Tourist Visa
Required Documents and Particulars
1. Passports
• Validity: Original Passport with at least 6 months validity and at least 2 blank pages.
• Old Passports: Original old passport booklets if any.
2. Photographs
• Quantity: 3 photographs.
• Specifications: White background; Glossy finish; 80% face coverage with a little shoulder visible; Eyes and face clearly visible; Dimensions: 45mm x 35mm.
3. Covering Letter
• Addressed to: The Visa Officer, Embassy of Czech Republic.
• Content: Brief profile of the applicants, purpose of travel, and travel dates.
• Format: Plain paper for individuals and company letterhead for officials/sponsored applicants.
4. Bank Statements
• Duration: Original 3 months personal and company's bank statements.
• Verification: Statements should be stamped and signed by the bank.
• Active Accounts: Should show proof of funds and income.
• Closing Date: Closer to the date of visa application (A4 size preferred).
5. IT Returns
• Documents: Latest 3 years Income Tax Acknowledgement / ITR-V (Saral) of individuals and the company.
6. Itinerary / Air Tickets
• Return Tickets: Return air tickets with all internal flight/train tickets if any.
• Tour Itinerary: Day-wise itinerary of the tour.
7. Purpose of Travel / Accommodation
• Proof: Any proof of your accommodation/purpose at the destination.
• Booking Certificate: Payment receipts from the tour operator.
• Hotel Vouchers: Required.
8. Supporting Credentials
Businessmen:
• Documents: Proprietorship certificate / Certificate of Registration / Memorandum of Association / Articles of Association / Partnership Deed.
• NOC: No Objection Certificate (NOC) from other partners or directors in case of a partnership.
Employees:
• Leave Sanction Letter: From the company (or government body in case of government employees/public servants).
• Salary Statements: Last 3 months’ salary statements (only for private firms).
• Company ID Card: Optional.
• Retirement Certificate/Letter: Copy of the pension book for retired employees.
Students:
• Leave Sanction Letter: From the educational institution (for travel during working days).
• Bonafide Certificate: From the educational institution (for travel during vacations).
• School/College ID Card: Optional.
Newly Wed/Honeymooners (if the spouse's name is not endorsed in their passports):
• Marriage Certificate: Original.
• Wedding Invitation Card/Photographs: In case the marriage certificate is not ready.
Children Traveling Unaccompanied by Both Parents/Either Parent:
• NOC: Notary attested No Objection Certificate (NOC) on a stamp paper from both/either of the parents respectively if both/either of them are not accompanying.
• Parents' Passport Copies: Copies of both/either parent’s passport.
Travelers Unaccompanied by Spouses/Partners:
• NOC: Notary attested No Objection Certificate (NOC) on a stamp paper from the unaccompanied spouse.
• Spouse's Passport Copy: If the spouse is not a passport holder, any government ID proof mentioning the spouse’s name can be submitted.
Additional Supporting Documents (if available):
• FD Receipts: From banks.
• Public Shares/Debentures Certificate: Required.
• Asset Evaluation Report: From a CA.
Travel Insurance
• Coverage: Valid Travel Insurance from the Embassy approved companies.
• Minimum Coverage: EUR 30,000.
• Validity: Should cover the days of travel.
