Tourist Visa 

To apply for a tourist visa for Poland, you will need to gather the following documents:

1. Passports

Original Passport: Must have at least 6 months validity and 2 blank pages.

Old Passport Booklets: If any, should also be submitted.

2. Photographs

Quantity: 3 photographs.

Specifications: White background, glossy finish, 80% face coverage with a little shoulder to be seen, eyes and face clearly visible.

Dimensions: 45mm x 35mm.

3. Covering Letter

Addressed to: The Visa Officer, Embassy of the Republic of Poland.

Contents: Should include a brief profile of the applicants, purpose of travel, and travel dates.

Format:

o For individuals: On plain paper.

o For officials/sponsored applicants: On company letterhead.

4. Bank Statements

Duration: Last 6 months.

Details: Personal and company bank statements with bank's seal and signature.

Relevance: Should show proof of funds and income from active accounts.

Format: Preferably A4 size, with a closing date close to the date of visa application.

5. Income Tax Returns (ITR)

Duration: Last 3 years.

Details: Income Tax Acknowledgement / ITR-V (Saral) of individuals and the company.

6. Itinerary / Air Tickets

Details: Return air tickets with all internal flight/train tickets if any.

Itinerary: Day-wise itinerary of the tour.

7. Purpose of Travel / Accommodation

Proof: Accommodation, purpose at the destination, booking certificates, and payment receipts from the tour operator.

Hotel Vouchers: Should be included.

8. Supporting Credentials

Businessmen:

Documents: Proprietorship certificate, Certificate of Registration, Memorandum of Association, Articles of Association, Partnership Deed.

NOC: From other partners or Directors in case of partnership.

Employees:

Documents: Leave sanction letter from the company (Government body for public servants), last 3 months salary statements (private firms), company ID card (optional), retirement certificate or letter, copy of pension book.

Pension Statements: Last three months.

Students:

Documents: Leave sanction letter from the educational institution (during working days), Bonafide Certificate from the educational institution (during vacations), School/College ID card (optional).

Newly Wed / Honeymooners (if the spouse's name is not endorsed in their passports):

Documents: Marriage Certificate (Original), Wedding Invitation card/Photographs (if marriage certificate is not ready).

Children Travelling Unaccompanied by Both Parents/Either Parent:

Documents: Notary attested NOC on stamp paper from both/either parent, passport copies/copy of both/either parent, birth certificate of the applicant, and copies of parents' ID cards (if they don't have passports).

Travelers Unaccompanied by Spouses/Partners:

Documents: Notary attested NOC on stamp paper from the unaccompanied spouse, passport copy of the unaccompanied spouse, or any government ID proof mentioning spouse's name if not a passport holder.

9. Additional Supporting Documents (if available)

FD Receipts: From banks.

Shares/Debentures Certificates: Public shares/debentures certificate.

Asset Evaluation Report: From a Chartered Accountant (CA).

10. Travel Insurance

Requirements: Valid travel insurance from Embassy-approved companies.

Coverage: Should cover the days of travel with a minimum coverage of EUR 30,000.

Ensure that all documents are properly arranged and meet the specified requirements before submission to avoid any delays in the visa application process.
