Tourist Visa

Important: As of 2 November 2015, biometric fingerprint data and a digital photograph are required at the Visa Application Centre with a prior appointment. Biometrics submitted after this date will be valid for the next 5 years.

Documents Required:

1. Passports:

o Original passport with at least 6 months validity and at least 2 blank pages.

o Original old passport booklets, if any.

2. Photographs:

o Two photographs.

o White background.

o Glossy finish.

o 80% face coverage with a bit of shoulder visible.

o Eyes and face clearly visible.

o Dimensions: 45mm x 35mm.

3. Covering Letter:

o Addressed to The Visa Officer, Embassy of the Slovak Republic.

o Should detail the applicant's profile, purpose of travel, and travel dates.

o Can be on plain paper for individuals and on company letterhead for officials or sponsored applicants.

4. Bank Statements:

o Original last 6 months' personal and company bank statements with bank seal and signature.

o Statements should be from relevant and active accounts showing proof of funds and income.

o Statements should be of a closing date close to the date of visa application (A4 size preferred).

o Credit card statement for the last 6 months.

5. IT Returns:

o Latest 3 years' Income Tax Acknowledgement / ITR-V (Saral) for individuals and the company.

6. Itinerary / Air Tickets:

o Return air tickets with all internal flight/train tickets, if any.

o Day-wise itinerary of the tour.

7. Purpose of Travel / Accommodation:

o Proof of accommodation/purpose at the destination.

o Booking certificate and payment receipts from the tour operator.

o Travel agency/hotel voucher including the name and address of the travel agency, registration number of the voucher, guest's name, arrival and departure dates, description of hotel services, and confirmation from the authorized person of the travel agency. If the voucher does not confirm payment, a separate invoice with the number, stamp, and signature of the bank statement confirming payment is required.

o First-time visitors to the Schengen area must submit the official Invitation Form verified by the Slovak foreign police department.

8. Supporting Credentials:

o Businessmen:

Proprietorship certificate / Certificate of Registration / Memorandum of Association / Articles of Association / Partnership Deed.

NOC from other partners or directors if applicable.

o Employees:

Leave sanction letter from the company (from the Government body for Government employees/public servants).

Last 6 months' salary statements (only for private firms).

Company ID card (optional).

o Pensioners:

Retirement certificate or letter / Copy of pension book.

Pension statements for the last three months.

o Students:

Leave sanction letter from the educational institution (for travel during working days).

Bonafide certificate from the educational institution (for travel during vacations).

School/College ID card (optional).

o Newly Wed / Honeymooners:

Marriage certificate (original).

Wedding invitation card/photographs if the marriage certificate is not ready.

o Children Traveling Unaccompanied:

Notary attested NOC on a stamp paper from both/either parent.

Passport copies of both/either parent.

If parents do not have passports, birth certificate of the applicant and copies of parents' ID cards.

o Travelers Unaccompanied by Spouses/Partners:

Notary attested NOC on a stamp paper from the unaccompanied spouse.

Passport copy of the unaccompanied spouse. If not a passport holder, any government ID proof mentioning spouse's name can be submitted.

9. Additional Supporting Documents (if available):

o FD receipts from banks.

o Public shares/debentures certificate.

o Asset evaluation report from a CA.

10. Travel Insurance:

o Valid travel insurance from an Embassy-approved company covering the days of travel with a minimum coverage of EUR 30,000.
